Living with pain can be debilitating. It can affect your work, your personal life, your sleep, your mood. If you have experienced pain for six months or more, we can help. We will identify your source of pain and provide individualized treatment options that may include minimally invasive procedures, physical therapy, lifestyle changes, education or a combination of these. Our goal is to avoid, reduce or eliminate opioid use, whenever possible, and refer patients for surgery only when necessary.
To become a patient of Pacific Sports & Spine, please request a referral from your primary care or referring provider. If you do not have a primary care or referring provider, please contact us to schedule an appointment, so that we can assess your condition.

What to Bring to Your First Appointment
• Your insurance card
• Imaging results (report and disc), if you’ve had imaging
• A current list of medications
• Personal identification with a photo (driver’s license of ID card)
• Insurance co-pay, if needed
Scheduling an Appointment
We do our best to get you scheduled as quickly as possible. If your insurance requires an authorization or if you need imaging prior to your appointment, these factors may delay scheduling.

What to Expect at Your First Appointment
When you arrive, please check in at the front desk and provide the receptionist your insurance card, imaging results, medication list and personal identification, as well as any additional documents that may have been requested. Please be prepared to pay your copay at this time, if needed. Once you are checked in, you are welcome to relax in our comfortable waiting room. When it’s time for your appointment, one of our medical assistants will escort you to an exam room and take your vitals. You will then meet with one of our physician assistants or your physician. After your consultation, our receptionist can assist you in scheduling your next appointment.
